It streams data, which means if somehitng needs to be fixed or something is going to be added, it happens when the Arena.net wants to send you the new stuff.
Edit: that's kind of vague...
they'll send new areas whenever they get them done, but I don't know what the timeline is, but I do know that if they need to fix somehitng, it can be done almost instantly as soon as aren.net is told of the problem.

I think they have allready updated the game, atleast there was a little disscussion earlier about a patch makeing the game unplayable, it seems to have been quickly fixed.
Probably MAJOR updates will be on expansions (new areas, new grapics, new spells) and other game "fixes" like tweaking armor values, weapon attacks, adding an npc, modifying some text, making monsters harder/tougher..etc.. will come in patches.
But your guess is as good as mine..

I wonder when are they going to start releasing the major expansions, the ones that have to be bought.

They have patched the game multiple times since release. They can patch the content without anybody noticing it, because of the instantiated design. Only new insances will get the new content. The rather small client is updated (if necessary) each time log on. If there is a new client when in-game, you will be asked (but not forced) to restart the client next time you enter a town or outpost.
Edit: The whole busniness model is based on regular expansion packs. Nobody knows about how often or at what price, except the price will be above US$ 20. They will be buyable online, probably also in retail.
I'm guessing every half year, and $35.

They patched it last night. not sure what it did but i think it changed the starting screen on mine. anyway it made the game stall after about 5 steps. they fixed it in like 2 mins so pretty good work!
Edit: they will definatly be brining out expansions but no one really knows how often. at least 1-2 a year i think!

All I had to do with the big update was log out and back in when a message popped up asking me to do so. There were a couple minor issues with the friends list and whispers, but it cleared up real quick.

They've already fixed some content issues as well: they changed the Wayward Monk quest from Yak's Bend(for monk primary/secondaries only), and fixed a bug where capturing a skill via a Signet of Capture did not unlock the skill for your character. Instead of having to live with these issues until a patch was released, they were pretty much invisibly fixed, within a day or two of being brought to attention. Considering how busy the devs are likely to be this soon after release, that's pretty good.
They've also shown that they can add content via streaming updates. Back in the E3 event last year, they added an entire mission halfway through the event- all that you had to do was restart your client when it asked you to, and you could access the new mission. They've said that they intend to release content like this periodically between expansions, though I wouldn't expect them to start putting out extra stuff for another month or two- they'll let everyone do the current content first.

The latest update moved the login screen to the right side lol. I started two new characters and there seemed to be more quests popping up than before. My W/N had two items on his quest list when I post-seared him at lvl 7.5. My ranger is at lvl 4(?) and has a grocery list of different quests. My lastly created mini-monk/ranger has found at least 20% more quests than the ranger and he is at lvl 5. The quests that never showed up for my Warrior all popped up for my last dude. Either the patches fixed the quests or I just missed them first time round.

Since they don't make a monthly income I think they will just pour out expansion after expansion so they can make some money.

Exactly. 1 or 2 expansions each year, with as much content as the original game - whats wrong with that?
